\n\n\n\nRadiation-induced injuries affecting urological tissues are a serious group of complications that may develop after radiotherapy to the pelvic region, usually for cancer of the prostate, bladder, cervix or rectum. Although radiation is directed at the tumor, it inevitably affects healthy tissues within the treatment field, including the bladder, urethra, ureters and distal rectum.<\/p>\n\n\n\n
These complications do not usually appear immediately. Chronic radiation-induced injury develops gradually, often months or years after treatment has been completed.<\/p>\n\n\n\n
It may present as haematuria caused by radiation-induced cystitis, dysuria, urinary frequency, narrowing of the urethra or ureter, fistulas between the bladder and the bowel or vagina, and severe fibrotic degeneration affecting bladder capacity and function.<\/p>\n\n\n\n
The underlying pathophysiological mechanism follows the classic triad of radiation-induced tissue injury: hypoxia, hypovascularity and hypocellularity. Radiation damages the vascular endothelium, reduces vascular density and leaves the tissue in a state of chronic ischaemia.<\/p>\n\n\n\n
The bladder mucosa, a tissue with high metabolic demands, gradually develops degeneration, telangiectasia, ulceration and fibrotic remodelling.<\/p>\n\n\n\n
A defining feature of this process is that the injury does not stabilize over time. On the contrary, it tends to worsen progressively, as vascular insufficiency aggravates ischaemia, ischaemia intensifies inflammation and inflammation accelerates tissue degeneration. Early intervention to interrupt this cycle is therefore critically important.<\/p>\n\n\n\n\n\n\n\n
Radiation-induced haemorrhagic cystitis, the most common and extensively studied radiation-related urological complication, is recognised by the Undersea and Hyperbaric Medical Society (UHMS) as an approved indication supported by strong clinical evidence.<\/p>\n\n\n\n
Multiple studies have reported response rates of 70% to 90%, with reductions in haematuria, improved mucosal integrity and substantial improvements in quality of life.<\/p>\n\n\n\n
For other forms of radiation-induced urological injury, including strictures, fibrotic degeneration and fistulas, the evidence is still developing but is based on the same established mechanism: restoration of oxygenation in irradiated tissue through neovascularisation and reactivation of cells involved in healing.<\/p>\n\n\n\n
In radiation-induced urological injuries, hyperbaric oxygen therapy acts as an adjunctive biological intervention that addresses the underlying vascular insufficiency, the central factor sustaining the various clinical manifestations.<\/p>\n\n\n\n
It does not replace urological treatment but may significantly enhance its effectiveness.<\/p>\n\n\n\n
Repeated hyperoxygenation helps restore biological activity in tissue compromised by radiation injury. It stimulates neovascularisation, improves mucosal integrity, reduces the telangiectasia responsible for bleeding and may slow fibrotic degeneration.<\/p>\n\n\n\n
A typical treatment course consists of 30 to 40 sessions, planned in close collaboration with the urologist and oncologist.<\/p>\n","protected":false},"excerpt":{"rendered":"
