\n\n\n\nOsteomyelitis of the jaw is a serious infectious condition affecting the cancellous bone of the maxilla or mandible. It is usually caused by bacterial pathogens that enter the bone through a dental infection, fracture or surgical procedure.<\/p>\n\n\n\n
The chronic form is particularly persistent. Bacteria form biofilms around necrotic fragments of bone, known as sequestra, creating a protected environment that is difficult for both antibiotics and the immune system to penetrate.<\/p>\n\n\n\n
Symptoms include persistent pain in the affected area, swelling, redness, difficulty chewing and, occasionally, discharge of pus through a sinus tract. In advanced cases, tingling or altered sensation may occur because of involvement of the local nerves.<\/p>\n\n\n\n
Diagnosis is based on clinical examination, imaging studies such as CT and bone scintigraphy, and analysis of any discharge to identify the causative pathogen.<\/p>\n\n\n\n
The principal characteristic of chronic osteomyelitis of the jaw is its limited response to antibiotic treatment. Medication cannot reach necrotic bone effectively because the affected tissue has no functional blood supply.<\/p>\n\n\n\n
At the same time, the hypoxic environment within the affected tissue favours the growth of anaerobic bacteria and impairs the phagocytic activity of white blood cells, which require adequate local oxygen levels to destroy bacteria effectively.<\/p>\n\n\n\n
The condition therefore becomes self-perpetuating: infection worsens ischaemia, while ischaemia allows the infection to persist. Breaking this cycle requires an intervention that addresses both factors simultaneously. This is precisely what hyperbaric oxygen therapy offers when used alongside surgical debridement.<\/p>\n\n\n\n
Chronic osteomyelitis of the jaw is one of the most extensively documented indications for hyperbaric oxygen therapy in oral and maxillofacial surgery.<\/p>\n\n\n\n
Clinical studies and case series have reported significant improvement in patients who received hyperbaric oxygen therapy alongside surgical debridement and antibiotic treatment, compared with similar groups treated without hyperbaric oxygen therapy.<\/p>\n\n\n\n
Its bactericidal effect against anaerobic pathogens, which results directly from hyperoxygenation, together with enhanced phagocytic activity and improved antibiotic penetration into the tissues, are the three principal documented mechanisms supporting the effectiveness of hyperbaric oxygen therapy in osteomyelitis of the jaw.<\/p>\n\n\n\n\n\n\n\n
In osteomyelitis of the jaw, hyperbaric oxygen therapy acts as a powerful biological adjunct to surgical treatment. It does not replace either surgical debridement of necrotic tissue or antibiotic therapy but significantly enhances the effectiveness of both.<\/p>\n\n\n\n
Each session creates unfavorable conditions for anaerobic pathogens, restores the local phagocytic activity of white blood cells and improves antibiotic penetration into the affected tissue.<\/p>\n\n\n\n
Over the longer term, neovascularisation helps restore the blood supply to the jaw, thereby reducing the risk of recurrence.
