Limb ischaemia is a condition in which blood flow to a limb, usually a lower limb, is reduced to a level that is insufficient to meet the metabolic needs of the tissues at rest.<\/p>\n\n\n\n
It is the most severe manifestation of peripheral arterial disease and may be classified as chronic or critical, depending on its severity and rate of progression.<\/p>\n\n\n\n
Critical limb ischaemia is a medical emergency. It is characterised by severe pain at rest, ischaemic ulcers or gangrene and carries a substantial risk of limb loss.<\/p>\n\n\n\n
The chronic form typically presents with intermittent claudication, meaning pain during walking that subsides with rest, and gradually worsens over time. Risk factors include smoking, diabetes mellitus, hypertension, hypercholesterolaemia and renal failure, which frequently coexist in the same patient.<\/p>\n\n\n\n
Limb ischaemia is characterized by a self-perpetuating cycle. Inadequate blood flow causes hypoxia, hypoxia activates inflammatory mechanisms, inflammation further compromises the vascular system and circulation progressively deteriorates.<\/p>\n\n\n\n
As a result, the tissues are unable to heal even minor wounds, while necrosis may spread both deeper and across a wider area.<\/p>\n\n\n\n
Treatment options are limited for patients who are not candidates for angioplasty or aortoiliac bypass surgery, either because of the anatomical pattern of arterial disease or because of coexisting medical conditions.<\/p>\n\n\n\n
In such cases, adjunctive treatments capable of improving tissue oxygenation without requiring invasive vascular reconstruction may be particularly valuable.<\/p>\n\n\n\n
Studies involving patients with chronic limb ischaemia have evaluated hyperbaric oxygen therapy as an adjunctive intervention, particularly in those who are unable to undergo vascular revascularisation.<\/p>\n\n\n\n
The findings have indicated improvements in transcutaneous oxygen pressure (TcPO\u2082), a direct measure of tissue oxygenation, reductions in pain at rest and, in some cases, healing of ischaemic ulcers that had not responded to conservative treatment.<\/p>\n\n\n\n
The stimulation of neovascularisation is considered the most promising long-term mechanism of action. The formation of new blood vessels within ischaemic limbs may improve long-term perfusion independently of the existing obstructed vessels.<\/p>\n\n\n\n
In limb ischaemia, hyperbaric oxygen therapy is used as an adjunctive treatment. It does not replace vascular revascularization when this is feasible, but it may provide an important additional option for patients who are not suitable candidates for invasive treatment or may be used as part of preoperative preparation.<\/p>\n\n\n\n
Its principal mechanism of action is to bypass the effects of vascular narrowing by increasing the amount of oxygen dissolved in the plasma. This allows oxygen to reach ischaemic tissues independently of haemoglobin and the narrowed blood vessels.<\/p>\n\n\n\n
At the same time, VEGF-mediated neovascularisation provides a form of natural biological bypass through the development of a new vascular network that may compensate for obstructed vessels over the longer term.<\/p>\n\n\n\n
